Keys to Your CareKeystone Mercy has Keys to Your Care programs to help you stay healthy. You do not need a referral from your PCP to participate in any of these programs.
You could also be identified to be part of one of these programs through claims submitted to Keystone Mercy by your PCP, specialist or health care provider. You may receive a call from a representative of Keystone Mercy to encourage you to participate in a program. Keys To Your Care has programs for the following conditions:
These programs help you better understand your diagnosis and heath care needs. As part of the program, you will be given educational materials (self-monitoring charts, resource listings and self-care tips), information about these conditions, and case management support to coordinate your health care needs. High-risk members will be assigned a nurse case manager. Your nurse case manager will work with you and your PCP to set and achieve personal goals to improve your overall health and quality of life. Information about your medical concerns is just a phone call away 24 hours a day, 7 days a week. As a Keystone Mercy member, you have the right to refuse to participate in a Keys to Your Care program. Refusal can be verbal or in writing to Member Services or to a Keystone Mercy representative. Refusal to participate in a Keys to Your Care program will not in any way change your Keystone Mercy benefits or the way you are treated by Keystone Mercy and its providers or the Department of Public Welfare.
