Keystone Mercy ID Card

Information on receiving your Member ID CardIf you have not received your card or if you have lost it, please call Member Services at 1-800-521-6860 (TTY 1-800-684-5505). A new card will be sent to you.

After you are enrolled in Keystone Mercy Health Plan, you will get a Keystone Mercy ID card. Your Keystone Mercy ID card is very important, keep it with you at all times.

Keystone Mercy Health Plan - ID CardYour ID Card has valuable information to help you get the health care you need when you need it, including your:

  • Name
  • Keystone Mercy Member ID number
  • Sex - Date of Birth
  • State ID Number
  • PCP name and phone number
  • Co-pay information (will be filled in specific to each member)
  • Keystone Mercy Member Services phone number - where you can get any question answered - and the TTY phone number
  • Mental Health Drug & Alcohol services contact information
  • Pharmacy services contact information
  • Additional information listed on the card is for health care providers

Pennsylvania ACCESS CardPennsylvania ACCESS Card

You will also get a Pennsylvania ACCESS card from the Department of Public Welfare. If you lose your ACCESS card, call your caseworker at your County Assistance Office. It is important for you to have both cards and to know when to use them. If you need to know where your County Assistance Office is located, or if you have questions about your cards, call Member Services.

Things to know about your ID Cards

  • If you have other health insurance coverage, you are required to present your medical card for that insurer at every office and pharmacy visit, along with your Keystone Mercy ID card and your Pennsylvania ACCESS card.  It is important that you carry ALL your health insurance cards with you at all times. You need all these cards for all of your PCP visits, specialist visits, prescriptions and other health care services.
  • You will need your ACCESS card for the Medical Assistance Transportation Program (MATP). You will need your ACCESS card for Behavioral Health Treatment (mental health/drug and alcohol.

It is important to carry all cards with you at all times.
